Between ages 21- 38 years old

Some clinics require BMI under 30
(Each RE sets their own guidelines on this)

Must not live in District of Columbia

Have given birth to at least one child that you are raising

Have health insurance that covers maternity

No pre-term births before 36weeks
(unless multiple pregnancy)

Must be STD free

No Drugs or Alcohol Use

Have reliable transportation

May not be receiving any government assistance

Able to provide your previous delivery records

Willing to undergo a psychological evaluation & written test

Surrogate & her partner must agree to all medical testing required by the clinic

Surrogate & her partner must be willing to undergo criminal background check

Must have a stable residence
